Nashville General Hospital at Meharry (NGH) is the only hospital in the country recognized by the Centers for Disease Control and Prevention (CDC) and the Office of Minority Health to lead a national pilot program to focus on those at-risk for chronic diseases. In 2007, NGH opened the Center for Lifestyle and Health Management, located in north Nashville, which serves as the model for this national program.
