Our program starts with a personalized health risk assessment profile which requires you to complete our Health History Questionnaire. We will then record your baseline measurements which include: height and weight, blood pressure, waist circumference and blood test results. Based on your assessment profile, we will create a Personal Action Plan that will outline short and long-term goals and methods to achieve those goals. Every client is assigned a personal mentor to assist them throughout the program.
